By default, as clinicians accept or add diagnoses for patients within the system, our intelligent platform learns their areas of focus. As a result, it automatically suggests relevant assessments under the 'Suggested Assessments' tab in our follow-up builder user interface. This intuitive feature is designed to streamline the process, saving clinicians valuable time when creating appropriate follow-up assessments.
In the following example, you'll notice that the clinician has accepted the following diagnoses for the sample patient:
Clinicom intelligently suggests appropriate follow-up assessments aligned with the diagnoses accepted by the clinician for future patient visits. These suggestions are tailored uniquely for each patient, taking into account their accepted diagnoses and previous assessments. While Clinicom offers these auto-suggestions to streamline the process, the clinician maintains full control over which follow-up assessments are ultimately sent to the patient. They have the flexibility to edit, modify, remove, or add to these suggestions as needed.
This feature serves to assist clinicians in focusing on relevant follow-up assessments while ensuring they retain diagnostic and follow-up autonomy. For instance, if a patient is diagnosed with Depression, Clinicom may recommend assessments such as HAM-D and PHQ-9 for effective follow-up. However, it's ultimately up to the clinician to accept these suggestions or make modifications according to their professional judgment.
You can add a follow-up assessment for your patient from either the All Results tab or from the Reports tab in the Patient Summary.
In the simplest follow-up approach, the clinician begins by accepting or adding a diagnosis. They then proceed to click 'Add Follow-up' followed by 'Send Follow-up,' having acknowledged the system's auto-suggestions. This streamlined process not only saves time for the clinician but also ensures that only assessments directly related to the accepted diagnoses, as reviewed in the initial Clinicom Report, are sent.
The clinician may remove any pre-selected scales from the follow up by clicking on the red “minus” button, then clicking OK to confirm the removal.
Additional scales can be added to the follow up assessment from the lists on the left side of the screen.
Here you can expand the lists to select additional scales and filter by condition.
When all scales have been selected, click on the Create Follow Up button.
You have 3 options in the Send To dropdown menu:
1) Send to Patient
This will send the follow up invite to the patient’s email address and/or mobile number in the account.
2) Send to Third Party
This will also send the invite to the patient’s email address and/or mobile number in the account. CLINICOM can only send communications to the patient. You may add a note to your patient to have them forward this message to any third party they choose.
3) If you select “Do Not Send Invite”, the follow up assessment will be created in the patient’s account, but no invite will be sent to the patient’s email address or phone number.
Click in the Send On field to select a future date to send the follow up assessment. By default, the invite will be sent on today’s date.
You can choose to have the same follow up sent on a schedule by clicking the Repeat dropdown list and selecting the number of times you would like to have the follow up invite sent to the patient.
In the example below, this patient will be sent the initial follow up with 5 repeat follow ups sent every 4 weeks.
Once a follow up schedule has been created, you will have a Follow Ups tab in the Patient Summary section. You may schedule additional follow ups for your patient if you choose. For example, you could click the Add Follow Up button as shown in the screenshot below and create an additional follow up schedule.
You would now see both follow up schedules for your patient. You can edit the schedule from the Follow Ups screen at any time by clicking the “Repeats” and “Every” dropdown lists, or you can delete a follow up schedule by clicking the trash icon.
